  Dear colleagues!I noticed that lately, more and more doctors are posting their clinical cases of complex work, using digital technologies in varying degrees.This work was performed on the clinical basis of the Department of Dentistry Postgraduate Education "Medical University" Reaviz "in Samara i with Alexander LysovSo, the initial situation is standard. Partial secondary edentulous on the upper jaw.Together with the patient, it was decided to rehabilitate the upper jaw with a `R2. In this case, the task was to restore the function and aesthetics on the day of the operation with a non-removable structure.Computed tomography was performed with a special teaspoon, prints were taken, which were converted to the STL format. By combining the data, we created a virtual Wax-Up, planned with Yuri Sedov in R2gateRussia the installation of 8 implants. After that, the template was designed in such a way that it was possible to install permanent individual abutments.And an imprint was made virtually with what appeared to be installed implants and sent to the digital laboratory by Vitaly Tetkin for the manufacture of permanent individual zirconium abutments and CAD / CAM bridges.On the day of the operation, the removal of the failed teeth, the installation of dental implants and the simultaneous installation of permanent individual zirconium abutments were carried out; the temporary milled bridge was fixed on the existing cement and ATTENTION !!! Bridge sat!     High aesthetic demands were placed on the forthcoming treatment as well as maximal sustainable result was expected by the patient. The following was decided: 1.Removal of teeth 1.1, 2.1 2.One-time dental implantation in the position of the removed teeth with AnyRidge implants using navigation guide R2GATE; 3.One-time installation of permanent zirconium abutments on the dental implants; 4.Provision of conditions for creating a buffer area in the coronal direction with the help of soft tissue plastic surgery by free connective-tissue auto transplants in the area of installed zirconium abutments; 5.Installation of temporary crowns on the fixed individual abutments.

  Placement of Dental Implants in the Comp..

  Patient K., 42 y.o., admitted to the dental clinic of the “Reaviz” Medical University with complaints about difficulties in chewing the food due to the missing 2.6 tooth. Following the examination it was identified that a capsule-shaped septum 7 mm in height exists in the maxillary sinus positioned in the medio-distal direction throughout the total length of the sinus bottom. The thickness of the alveolar crest in the projection of the 2.6 tooth is 5 mm. The obtained data made it obvious that the standard sinus lift procedure would yield no positive results. Therefore a decision was made to perform the operation using the Samuel Lee’s patented  procedure and the crestal sinus lift MICA Kit by MegaGen.

  Expanding the Indications for the Immed..

  Case StatementA female patient B. admitted to the dental clinic of the “Reaviz” Medical University with complaints of difficulties of chewing the food due to lack of fixation of the bridge-type structure of the right maxillary. Examination DataFollowing removal of the orthopedic construction it was identified that  tooth 1.7 has indications for extraction (root caries, fracture in the mesio-distal plane), tooth 1.6 is missing. The abutment tooth 1.5 is in good condition.X-Ray Examination DataFollowing the study of the CT data in the position of the 1.7 tooth counter-indications were identified for the placement of the implant immediately after the extraction of the tooth. Primary stability of the implant seemed impossible due to the following reasons:1.The bottom of the peridental socket is directly adjacent to the maxillary sinus; implant stability is impossible in the periapical tissue;2.Placement of the implant in the interradical septum is impossible (septum height: 6 mm, thickness: 2.5 mm max);3.Stabilization of the implant in the peridental socket walls with conventional implanting systems is not possible; max diameter of the socket is 6 mm.   Therefore, the use of the conventional implant systems would require delayed implantation which would increase the treatment term (3 months) and the number of invasions.Treatment Plan1.Place the AnyRidge Ø 5,5x10 implant in the anodentia zone;2.In the position of the tooth 1.7 perform immediate implantation using the AnyRidge Ø7x10 dental implant. Sufficient primary stabilization of the implant is planned to be achieved from the aproximal walls;3.In order to mitigate the soft tissue defects following the extraction of the tooth and to increase the gum volume (form the tissue barrier) it was planned to use the epitelial autotransplant from the palate area on the pedicle.4.Following the osteointergation process, place the healing abutments. 5.Orthopedic rehabilitation using customized titanium abutments.
